Our Individualized Support services and professionals are available in virtually any setting, whether that be in the individual’s home with family or roommates, or in one of our residential settings. Through this service, our recipients are able to work toward personal success.
A goal for our entire organization is to ensure each of our services is as individualized and customizable as possible.
We work to build support and services to their individual needs wherever they’re at. We want to help them reach their chosen goals and the highest level of independence possible in whatever capacity that looks like to them.
We work to provide individuals with an additional layer of customized support. Trained support professionals are able to adapt supports to personalized settings whether the individual is living on their own, with family, with roommates, or in another group setting.
Our primary goal with this service is to help recipients assert their independence. Because we operate with a person-centered approach, they’re in the driver’s seat. They set their goals and families, guardians, social workers, doctors, and/or other service providers contribute to plans and reports that convey the necessary supports to attain these goals.
Positive Supports are preventative and strategic services we provide to our service recipients that can be provided in-home as well as in an apartment or licensed setting.
This support strategy ensures that each of our service recipients are getting adequate care based on their unique needs while remaining considerate of their culture and background. Through assessments, planning, implementation, and re-evaluation, our trained teams can provide them with the right support.
No two people are going to require the same type or level of support. Based on the individual’s unique needs, our trained teams will be able to strategize a preventative plan that will help our service recipients be successful within their homes and chosen communities.
Support professionals are trained in de-escalating situations without the use of confining methods. In response to behaviors, Direct Support Professionals (DSPs) are taught to ask who, what, where, and how in order to apply knowledge that can lead to a why. Through this strategy, new approaches can be planned and put into effect.
Person-centered in nature, this support adopts a holistic and evidence-based approach to preventing problem behaviors. With this service, strategies for addressing these behaviors are frequently being adapted and reworked in order to engage the source of an individual’s behavior. Through time and specialized assessment from certified professionals, our trained team is able to ensure that each person is receiving the best possible support.
Something we’re passionate about is supporting families who take on the work of helping people in need of disability support services. We truly believe that sharing life with someone in a smaller, more personal setting is the best approach for support. WFC has always operated to improve the services available to people with disabilities in our community and advocate for as many quality family service providers available as possible and will continue to do so as we move forward.
Our Foster Family Support services are available to people who want to make their home available to someone(s) in need of support.
WFC doesn’t provide this service in affiliation with the government or foster care system. Additionally, all license holders must have a Designated Coordinator and Designated Manager (DC/DM) according to Minnesota Statute (245D.081). We’re available to providers, or those looking to become a provider in northern Minnesota, in need of this service.
There are a variety of ways that we work to support people looking to foster individuals in a smaller setting. This includes assistance with compliance, paperwork, licensing, training, and more.
We don’t simply want there to be as many family providers as possible. Our goal is to support individuals in their venture to create these homes to provide high-quality, individualized, goal-oriented support so they can be as successful as possible.
Everyone is provided with an accommodating room and our highly trained and qualified Direct Support Professionals (DSPs) provide the individualized supports needed in service recipients’ daily activities. We work diligently to provide assistance within an individual’s living space that’s customized to each unique person.
CRS provides residential services in licensed settings, which includes day-to-day support that’s particular to each person and determined by need and their distinct objectives.
Our DSPs strive to get to know our residents – their likes, dislikes, preferences, and potential triggers. Residents often join us with a series of things they enjoy. As service recipients go through the transition of moving to a new place, our DSPs help them adapt to a new setting and help them find new enjoyments.
Some people with a disability require long-term care from trained professionals and it can be difficult and expensive to provide that resource from home. With 24/7 support from DSPs, individuals may receive assistance with scheduling, transportation, medical and employment appointments, and any other activities they have scheduled for the day.
